Polypharmacy Patient Survey

The NHS Integrated Care Boards (ICBs) replacing Clinical Commissioning Groups (CCGs) (the organisation that plans and buys health services) would like to hear from people living in Leicester, Leicestershire and Rutland about how they feel about taking medication and particularly from those who take five or more medications, known as Polypharmacy.

The insights we receive will help us to:

• Reduce patient harm from inappropriate medication use
• Help us to manage complex patient cases and inappropriate medication use
• Reduce the waste of medications resulting from inappropriate use

Your views are important to us and will help us understand the current and future needs of the local population.

The survey closes on Sunday 11th September 2022.

Measles is extremely infectious and can be serious. If you have symptoms of measles, please stay at home and phone your GP or NHS 111 for advice. STAY AWAY from GP surgeries and A&E departments – you could spread the illness to others.

PRACTICE GUIDANCE: COVID-19

Mask wearing guidance has changed for healthcare settings.  Staff will be required to wear face masks in clinical areas.

Patients are advised to wear a face mask when attending the Practice.
