Clinical Pharmacist

37.5 hours per week

Latham House Medical Practice is looking for an experienced Clinical Pharmacist to complement the existing team of Pharmacists and Technicians.

Working alongside our current Pharmacy Team the post-holder will quickly become an integral member of the Practice and enjoy a diverse range of duties and responsibilities including:

*Resolving medicines related queries with both healthcare professionals and patients.

*Further develop the Practice’s strong working relationships with care homes and community nursing supporting the management of patients.

*Conducting medicines/polypharmacy reviews either in a face-to-face clinical setting or by telephone.

*Developing their clinical skills in a chosen area of expertise.

*Contributing to and supporting the Audits and Quality Improvement Programme to ensure prescribing is aligned to the local formulary.

*Ensuring the achievement of targets and budgets.

*Liaising with Pharmacists working within the Leicester, Leicestershire and Rutland ICB

As a self-motivated and enthusiastic Clinical Pharmacist, the successful candidate will enjoy:

*Candidates will either be non-medical prescriber or will work towards this.

Are you a dynamic and experienced Diabetes Specialist Nurse looking for an exciting opportunity? Latham House Medical Practice, an enhanced Practice for Diabetes, is seeking a dedicated professional to lead our diabetes team and deliver high quality services to our practice population.

As our Lead Diabetes Nurse, you will play a crucial role in ensuring the delivery of high-quality, evidence-based diabetes care. You will work autonomously, using your strong professional decision-making skills and clinical expertise. Excellent communication and interpersonal skills are essential to support and empower our patients in managing their diabetes effectively.

Measles is extremely infectious and can be serious. If you have symptoms of measles, please stay at home and phone your GP or NHS 111 for advice. STAY AWAY from GP surgeries and A&E departments – you could spread the illness to others.
