Active Together Partnership Programmes

Active Referral

Active Referral is a selection of physical activity programmes for people with medical conditions, whose health would benefit from being more active. Each programme helps people to be active in a safe environment and manage the symptoms of their condition.

The following programmes are available under the Active Referral Scheme:

*Please note there is typically a cost to access these services in Leicestershire. Please contact local co-ordinator for full details

Activities will differ depending on the programme and location but typically include:

  • Fitness/gym based personalised exercise programmes
  • Swimming pool-based activities including aqua aerobics
  • Low impact group exercise classes such as Tai Chi, seated exercise or circuits
  • Group health/guided walks
  • Green gym projects such as community allotments and conservation projects
  • Virtual physical activity sessions where appropriate

Measles is extremely infectious and can be serious. If you have symptoms of measles, please stay at home and phone your GP or NHS 111 for advice. STAY AWAY from GP surgeries and A&E departments – you could spread the illness to others.

PRACTICE GUIDANCE: COVID-19

Mask wearing guidance has changed for healthcare settings.  Staff will be required to wear face masks in clinical areas.

Patients are advised to wear a face mask when attending the Practice.
